Having an organized, compassionate, and knowledgeable discharge planning team is at the heart of any great Detox, and this is where Raegena Phelps and her team come in.
When a client arrives, Raegena and her team go to work. They begin by speaking to the client and finding out what they want and what they may have tried or not tried before. Based on the client’s desires, schedule, Insurance, and resources, they put together a list of options for the next level of care.
The team will contact facilities, make introductions, talk to the client about what to expect, and organize transportation to make sure the client arrives at their chosen destination safely.
